MDH’s Afya Jumuishi is a five-year (2021-2026) project aimed at supporting the Government of Tanzania to provide comprehensive HIV and TB prevention, care and treatment services. The project is implemented across three regions of Dar es Salaam, Kagera, and Tabora, in collaboration with the Ministry of Health (MOH), the President’s Office of Regional Administration and Local Government (PORALG), and the corresponding Regional and Council Health Management Teams (R/CHMTs). The project supports 573 health facilities and 443 wards across all 21 councils in these three regions.

The Afya Jumuishi Project contributes to Tanzania’s mission to end AIDS as a public health threat by 2030 in line with the UNAIDS 2030 agenda. Supported services include:

HIV Testing and Prevention Services, which focuses on ensuring that all people living with HIV know their HIV status and are linked to lifesaving anti-retroviral treatment. Individuals who are HIV-negative but at risk of HIV infection are offered effective HIV prevention services for as long as needed to keep them free from the infection. These prevention services include Pre-Exposure anti-retroviral Prophylaxis (PrEP), Condoms, Prevention of mother-to-child transmission of HIV (PMTCT), Voluntary Male Medical Circumcision (VMMC), Interventions for Adolescent Girls and Young Women (AGYW), Gender-Based Violence Prevention, Medication-Assisted Treatment (MAT) for individuals who use drugs and others. In 2024, through the Afya Jumuishi project MDH’s work reached 1,338,719 persons who were tested for HIV, 37,147 at-risk persons offered PrEP, 19,205 pregnant women offered PMTCT services,117,710 men circumcised and many others.

Comprehensive HIV Care and Treatment focuses on ensuring that all people living with HIV, also known as recipients of care (ROCs), are provided with and sustained on effective Anti-Retroviral Treatment (ART) and supported to adhere to and remain on lifelong treatment. MDH supports differentiated service delivery models (DSDM) such as multi-month dispensing of anti-retroviral medications and Community ART. We also promote a holistic approach to care that puts the ROC at the centre and integrates comprehensive health prevention and Management including Advanced HIV, Non-Communicable Diseases, TB, Cervical Cancer and other conditions. All ROCs are tested for Viral load at least once annually and are supported to achieve sustained viral suppression and Enhanced adherence support. In 2024, through the Afya Jumuishi project, MDH contributed to enhancing the lives and livelihoods of 331,362 ROCs who received ART care.

Health System Strengthening focuses on contributing to ensuring that core components of the healthcare system, such as Laboratory services, Supply Chain Management, Health Information Systems, Human Resources for Health (HRH), and others, are well equipped to support comprehensive HIV and other healthcare services.

This comprehensive approach ensures that the needs of people living with and affected by HIV are effectively met across the targeted regions.
